Medicine Matters

Why a Current Medication List Could Be One of Your Best Health Tools

When was the last time you wrote down every medication you take?

For many people, the answer is “I’m not sure.” That’s understandable. Between prescriptions, over-the-counter products, vitamins, supplements, eye drops, inhalers, creams, and “as-needed” medications, it can be easy to lose track.

But having a current medication list is one of the easiest ways to protect your health.

A medication list helps your pharmacist, doctor, specialist, or caregiver understand exactly what you are taking, how often you take it, and whether there may be duplicate therapies or possible interactions. MedlinePlus recommends including prescription medications, over-the-counter medicines, vitamins, supplements, and herbs on your list.

What should be on your medication list?

A helpful list should include:

Your medication name, strength, dose, how often you take it, why you take it, and who prescribed it. Be sure to include over-the-counter products like allergy medicine, pain relievers, sleep aids, antacids, and cold medicines. Supplements and herbal products matter too, because they can sometimes interact with prescription medications.

You should also list medication allergies or medicines that caused problems in the past. This can help prevent repeat reactions or confusion during urgent care visits, hospital stays, or new prescriptions.

Why it matters

Medication errors can happen when information is missing or outdated. The CDC notes that medicines, vitamins, and supplements all carry risks, and adverse drug events may include side effects, allergic reactions, overmedication, or medication errors.

A complete medication list can help answer important questions, such as:

“Am I taking two medicines that do the same thing?”
“Could this new prescription interact with something I already take?”
“Is this dose still correct?”
“Do I still need this medication?”

These questions are especially important if you see more than one doctor, recently left the hospital, started a new supplement, or care for a parent or loved one.

When should you update it?

Update your list any time a medication is started, stopped, or changed. Bring it with you to doctor appointments, pharmacy visits, urgent care, the hospital, and even dental appointments.

It is also smart to keep a copy on your phone and a printed copy at home. Caregivers should know where to find it in case of an emergency.
